According to Polygon, franchise game director Alex Cox and associate design director Josh Zammit detailed how Man of Honor serves as a crucial narrative link between different eras of the Mafia universe. The base game established Enzo Favara, a young laborer who joins the Torrisi family in early 1900s Sicily. The new chapters slot into a one-year time jump within that story, focusing on Enzo’s encounter with a younger Ennio Salieri.

Connecting Eras and Character Arcs

The DLC dives into events preceding the rise of the American crime kingpin known from Mafia and Mafia 2. Zammit noted that these chapters reveal aspects of Salieri's life that players of previous games would not know. His primary motivation during this period is his desire to secure a path to America, an ambition that ties directly into Enzo’s own aspirations for leaving Italy.

The Dynamic Between Protagonists

Cox describes the relationship between Enzo and Salieri as a study in contrasting personalities.
